Index and Ika-Musume . They are so similar, it is officially admitted.

I think Tsunayoshi "Tsuna" Sawada of Reborn! & Sena Kobayakawa of Eyeshield 21 look almost similar.

Lets face it, anime (and all cartooning) has a limited number of different ways to draw a character. Plus, there are strong archetypal looks and classic personality types in anime - if you want to portray a personality, its faster shorthand to draw from those archetypes than to do something unique.

Minami Iwasaki and Yuki Nagato. Hair color aside af course. Having the exact same voice helps as well.

Ayanami Rei and just about every other Emotionless Girl.

Haruhi and Yurippe (AB!) are similar.

Saber(FSN) and Sylvia(Princess Lover). Not just both had similar hairstyle, same hair and eye colors but also both had excellent swordmanship though Sylvia is mora at fencing.

Hibiki Houjou and Yui looks alike despite their hair colour and eye colour being different Also Yuuko Aioi and Itsuki Myoudouin looks similar too, oh yea Amber and C.C.

Minami Iwasaki and Hideyoshi Kinoshita do bear a similarity that's just separated by a palette/gender swap.

Another case could be made for Ami Kawashima and Meme Touwa being just an 'age up'.

Of course there is the case of Rise K and Nena trinity to consider as well.

They look so alike its scary.

Let me tell you guys one thing, all girls with long black hair in anime, manga and game are similar. If you compare each of them side by side, I bet they are like clone with diffrent art style. Girls wit hime cut is the same as well.

Hahaha! I find this funny since I'm watching Seto no Hanayome right now for the first time and recently noticed the similarity before I read your post. I didn't see anything familiar about Luna when she was introduced due to the difference in art style but this particular shot showed a glaring similarity to Madoka that flipped my brain:
