Yeah, the three conditions are just to become Center Nova, and once you maximize your "radiance" afterwards you disappear. It just so happened that both occurred within the same few minutes for Yuuko.
Presumably Acchan qualified as CN at a concert that hasn't been talked about, and simply disappeared later when she'd accumulated enough idol power.
