it's quite easy to turn a hollow into an arrancar, just rip off parts of his mask(grand fisher's first transformation for example)
and it's no secret that natural(imperfect) arrancars have been around before the hougyoku was in Aizen's hand:
Noitorra said "how many years have passed" when Nell turned into a woman. That makes Noitorra an natural arrancar who got tuned by the hougyoku afterwards("Aizen gave me power" and his Zanpaktou's form changed too). Stark/Lilynette already looked humanoid when Aizen found them.
