I think a permanent change to his appearance to signify the extent of the change would be good, but whether he'll change to completely permanent vampire looks is another thing.
One thing to remember is that both Akasha and Moka before the seal did not permanently show all of thier vampiric traits. I think a partial change, such as say skin colour and teeth or hair would be enough for Tsukune, whilst leaving the eyes as more human, except when he is unleashing his youkai or extremely stressed/emotional. It'd also make it easier to hide his growing inhumanity when dealing with humans; teeth can largely be hidden, as can hair, whereas eyes... well permanent contact lens would be a bit of a pain. I'm actually dying for the eventual next face to face/unveil to his parents of the entire situation. The girl he's in love with and his cousin supports is a vampire; he's no longer human and is basically a damphir/demi-vampire...which he has more or less accepted and actually thanked her for as its kept himself and those he cares for safe; and he is now as much a part of the world of the supernatural as much as the human one.
Should be one hell of a potentially cool (and possibly funny) chapter.
I heard a while back that Ikeda once worked under Norihiro Yagi of Claymore fame, and it works a similar way in that in terms of eyes for the Claymore. They're also human turned something else, so given his familiarity with the series (I would find it hard to believe he'd be completely ignorant of his mentor's current ongoing work) it could be a visual cue that Ikeda picks up. It's not a perfect scheme, and hell, permanent vampire eyes would be cool too, but it wouldn't be a surprising route to take, and would perhaps be easier for Tsukune to manage.
That said I don't want NO physical changes, I think something needs to happen to signify the change in Tsukune's makeup; previously you could consider him as still 50:50 human to ayashi, whereas now it's more like 90:10, a human core and mindset residing in a virtually Ayashi body.
Providing Tsukune manages to return to human form with a 'touch of badass', Moka might quite like the change... O.M likely won't care; she already openly admits she's in love, and it might be a push for I.Moka, as not only will it signify that Tsukune himself is willing to change and fight for her, but also in changing, he is making himself stronger and thus might not only assuage her pride, but also her fears that spending too much time with him will make her weaker
