Kihara was a one off, as the scientist that studied him since the begining, he had full understanding of his powers which I assume includes the timing of his reflection. It's the same as how telestina knows that Mikoto's coin railgun can only go up to 50m.
As for Touma, pretty soon I'm sure he can dodge bullets...
