Arclight ordered him to only kill Blade, because he suspected Yamada might have what he was looking for, hence why. Besides, after Blade released himself of his strings, Kafka really went for the kill, only to find Eve instead. Nothing wrong with that.
Speaking of Kasumi, her body isn't even functional anymore, but the half of her brains could be used to bring her back by transplanting that part on the body of someone else. Though it wouldn't exactly be Kasumi.
