Well, "tensai" literally applies to everyone they use it for. The implication is to be derived from the context, and for everyone except Lee, the term is applied as it should. In Lee's case, it's a bit of a stretch.

So contentually breaking down "tensai" into English:

Shikamaru is a genius.
Sasuke has lots of raw talent. A good pedigree, I suppose.
Neji is a prodigy.
