I am pretty sure, after all the happy moments between Noe and Shin in this episode, the next one will be either the climax or start going downhill for Noe X Shin. If Noe is going to end up with anybody, it will probably be just her brother.

Who's going to pay for it in this small town? Is there a doctor who can perform it in this small town? Either way, if they do get it, it would be announcing to the world that they think they are siblings. I think their parents would need to get involved, so I don't think it is a viable option for them right now.
