That would be "Dear Kaname Madoka." The circle says, "What Saitou Chiwa-san imagined Homura's Valentine's message would be" and the card underneath (I assume a quote from Saitou Chiwa herself) reads "She's the type to overthink it and end up unable to properly say what she wants to say (laugh)."
