There's a manga called "Life" where all these terrible things happen to the main character like you described, but it's set in modern Japan and not another world or anything. I've just read about it because I'm very averse to these sorts of plots.
To be honest, I think it's too pessimistic and one-sided about humans, but I can see how it's interesting to see the absolute worst potential of humans.
Sorry, I don't know any other animes like that off the top of my head. To be honest, I didn't think Attack on Titan was like what you posted, though the titans themselves were really brutal. I haven't finished it yet though so if all that other stuff comes up, I wouldn't know.
There's a visual novel called G-Senjou no Maou and I think you would like that, maybe ^^ It's a psychological thriller, by the way.
