because your description matches the best game in the world which currently doesn't exist

you can't have everything that appeals to everyone

quick payoff quests dont work because anyone with decent gaming ability would finish all the content in less than a week and have nothing else to do

same with less grinding. you finish the content, then what.

innovative and interesting is a point of view. most mmos try to update their contents with patches so almost all mmos fit this category to keep players interested
