In which case it would be a bittersweet ending, instead of one with a measure of closure and continuation.

With Price (Frost and Nikolai also survived the war) outliving the entire affair, there was actually a glimmer of hope, of closure - that the entire thing could be put behind oneself and one could walk forward...possibly into new worldwide hiccups, but away from the previous one. Killing everyone off would be quite disheartening IMHO -- notions of poetic justice, appropriate symbolism or beauty of the situation shoved aside.
As for Gen. Shepherd's conspiracy, it's already dead and buried. Even if Soap and Price had both survived, there was little chance the truth would get out, since most actors are dead at the end of MW2 and, with the Russian assault setting the entire world on fire, Shepherd's disappearance was conveniently overshadowed. There would be no point in publicly rectifying the story since "Shepherd's Truth" (in reference to the MW2 dialogue where "his lies become the truth") never had the chance to become reality. He died before he could be hailed a hero and the evidence (the DSM module Task Force 141 had been ordered to retrieve from the mountain villa) is probably buried in the collapsed caves deep down in Afghanistan.
Trying to reveal the entire story would only cause more problems to the ex-141 fugitives since certain parties would only be too happy to settle the score, this time with further justification...courtesy of the leaked info. A blame game that's just started doesn't end that quickly and Shepherd's little secrets would only add oil to the fire. I doubt Price would not realize that and acknowledge that the only thing he could do would be to shut up and deal with his current state of disgrace.
Highly improbable. They'll probably create an alternate universe set in modern times (not MW timeline) or they'll settle for a new threat (China? North Korea?) with a new cast. I just hope they won't go the Ghost Recon way, with their Future Force Warrior interpretation - I'd rather they stick to current hardware and timeframe.
