Assassin is appearing earlier in the Ep 4 OP because...there are actually 4 Assassins there!
If you look carefully, in the first 3 episodes you see Saber swinging at nothing before clobbering Assassin. In the Ep 4 Op, rather then swinging at nothing she is knocking away two other Assassins, and then when she swings to knock the obvious Assassin out of the way another one jumps in and gets hit as well.
I am disturbingly experienced in watching that OP :x
