It is not weird for people to have the same surname especially in Japan.
I think Miyuki will accept the person that Tatsuya is going to love in the future even if it is not her. Cause she knows how much Tatsuya suffers and he deserves to be happy even if the person standing next to Tatsuya in the end is not her. Cause Miyuki thinks she is not worthy of being Tatsuya most important person and Tatsuya has no choice but think that she is because of the brain washing. Allow me to quote one of my favorite lines ever "I love you but please don't love me" is what Miyuki thinks at the moment.
