#1: Read the TIPS "Schrodinger's Cat" and "Vain Island". They may shed more light on the answer you're looking for.
#2: It was explained why in Izumi Cure why Izumi had her memories- because she saw Makoto on the verge of death, and so she wished to go back and change things, just like Makoto did. Her Cure Syndrome activated, and so she went into her own delusion, sharing it with Makoto... possibly.
Yuka's is a bit more complicated. My assumption was that Yuka was also afflicted with Cure Syndrome (and that the game "Never7" was actually Makoto and Yuka's simultaneous delusion), and so she had time traveled before Makoto did (hence why she bursts into his room in every route), but the information in the TIP "Schrodinger's Cat" contradicts this... except for in the Izumi Cure route.
I'd recommend reading Yuka Cure. There's a lot of new scenes, and they help do two things- explain Yuka's feelings for Makoto in greater detail, and give some more backstory on the boy Yuka loved. To see the latter, however, you have to go look for the bell first on Day 3.
