If you look at the spolier then you can see what I wrote about what I know about Naruto's family. As far as we know Tsunade is not Naruto's gradmother, though who is mother's family is is still unknown.

His mother is unknown to us they don't really talk about her that much. As far as we know he as no other family members then the his mother father and Godfather."
