I've been checking its past works and, (though I might not be looking hard enough), I think this is the first time SUNRISE has done something to give an impression that the main character is dead.

Kamille Bidan in Zeta Gundam (not clinically dead, but braindead for a while)
Gai Shishioh (Gaogaigar Final)
2 of the pilots of Zambot 3 (Zambot 3)
Cosmo Yuki and everybody else in the show (Space Runaway Ideon)
Sho Zama and almost every other character (Aura Battler Dunbine)
Spike Spiegel (Cowboy Bebop)
Roger Smith (Big O, because I can't really call what he has a life "living" after we find out the truth of Paridigm City in the end)
Robin Sena (Witch Hunter Robin)
Akira Hibiki (Planned: Originally Akira was going to die as a result of using the incredibly poweful God Voice)
Lelouch Vi Britannia (Code Geass.....duh!)

Some of them I play fast and loose with the definition of death, but meh, I'm not the one to decided the one true definition of death so it's up to other how to these scenarios.
