Personally I think he comes up with ideas on the fly and pins them to a dartboard to decide which ones to use.

Nisio went out of his way to explain how IIhiko was just like Medaka in that he was born lucky, a monster who the world revolves around making him victorious in everything. I think we were all expecting Medaka to pull some new power out of her ass and win against this new villain, so much so that we forgot he wasn't actually a villain. That Medaka was out to save Shiranui from something she didn't actually need saving from, in the end this was just Medaka forcing her views on someone else. Ajimu said that in a fight in a manga, the more heroic one wins, but just being Medaka's opposition didn't make IIhiko evil by default.
So she loses.
I can't wait for the next chapter. It's bound to be Fresh!!!
