yes the cake was made by his sister, but as you can see the was destroyed by ayano when his sister visiting to say happy birthday. In this volume we can see that ayano was very scary and almost like a very obsessed stalker because when she was asked that if ayano came to see our protagonist or not, she just answer without remorse or anything that nobody is coming to see him at all that morning.(the morning where his sister come and want to said happy birthday to her brother)
