This is basically the end of the PS2 version, where the epilogue is extended:

There is a scene where Tomoya woke up after he fall in coma, as result of the surgery.
Tomoyo waited for him all this time, etc.

Either Tomoya is alive, or it is a big metaphor that they meet in afterlife or so (rather the first option
