Its ok, for the column that says "一番", check the button for "イリヤ". Next for "二番", select the button for "セイバー" if you want Saber or "遠坂凛" for Rin. Then for the box for "メールアドレスを入力してください。
(二重投票のチェックと、プレゼント当選時のご連絡に使用させていただきます)", make up an address. and for "上記のキャラクターを選んだ理由をお願いします。(任意です)" is supposely for why you chose what you chose, so write bad Engrish.
Though IMO, for people who haven't played the game, I think you shouldn't vote until 2/10 for the sixth episode to atleast see more characters before voting Saber/Rin since they're going to win again anyway.
