I retain the hypothesis that Ichigo is Kaien's reincarnation, the hollow within him coming from dying while fused with Aizen's monster, NOT gaining them due his Shattered Shaft experience. Maybe it was strengthened due to that, but we saw examples of Ichigo having unusual spirit power and abilities well before he lost and regained his powers.
As far as Isshin goes, he can't be Kaien because of the way Kaien died: pierced through the heart, I thought it was. I'd say it's pretty clear he was a captain, but I'd also wager "Isshin" isn't even the first/given name he went by in Soul Society.

P.S. REIATSU. I've never seen a translation outside this forum that uses an "L" instead of "R" and don't bother telling me it's interchangeable. Official translation by way of Viz is "Reiatsu."
