The girl is, indeed, Naru...they hinted to this several times towards the end of both the original series and the LHA OVA set. The fact that Mutsumi said that she overheard Keitaro saying the promise is a dead giveaway. Coupling this with the origins of Naru's favorite stuffed animal and you have a pretty good idea as to who the girl really is.

I think they did a good job at twisting it into making you really think at who the girl really was though.

It's sooo damn confusing. the confession occurs, and Mistume gives the doll to Naru, BUT NARU'S NOT IN THE SANDBOX, she's even wearing a diff outfit.

even in the xmas special, Motoko's depress about the Keitaro and Naru's love.

but MAN, Motoko's dream in episode 08 really throws me off.
