Wrong Rider, you're talking about G. Kamen Rider in question in that image is Decade, aka dimension-travelling Rider that can use the powers of other Riders and other main characters, and in this case Decade uses a Final Form Ride on Nanoha, i.e.
transforming a character into a signature weapon, to turn her into a massive Raising Heart. The process is... kinda freaky as you can tell
