Well, the killer had psychological problems, even when he was a child, so this is beyond how the parents can teach their children, or how the current state of the society is kind of thing. And, considering that, if the system didn't have the defects that you mention in your post, that specific event wouldn't have happened. Although it won't change the fact that, another one might have come up with another massacre, still, this one was within the reach of the system's control, and it failed to do so.
