NTR has nothing to do with Mind-breaking...you only see that in hentai manga.
NTR is a matter of view point:
The MC is in a relationship with a girl. That girl is then stolen by some other guy (could be from many reasons, from her already being in love with him, to the guy wooing the girl or the more extremes you see in hentai involving being forced and ending up mind-broken).
To the MC it's Netorare (He's the guy who's girlfriend was stolen by another).
To the girl it's Cheating (She's cheating on her boyfriend with another guy).
To the guy it's Netori (He's the guy that stole another's girlfriend).
Note that if the stolen one does not appear to have feelings (love or lust) for the new one, it is NOT NTR or cheating as she's being forced into such a situation.
