what exactly happened when Ichigo turned into a bad ass on Ulquiorra. He just kind of snaps out of it and it's never explained what was going on. We don't even really get Ichigo's feelings on what was going on as if he wasn't doing any sort of inner battle. He was acting like someone just woke him up as he was failing asleep in class.

It was probably just that, he felt like it was a bad dream. Literally he was dying when Orihime cried out to him. His "instincts" really kicked in maximum overdrive to protect her.
