Spoiler for Not really a spoiler at all but the original requirements were in one so I'll follow suit:

I had her unlocked right at the beginning when loading my cleared data, without even realizing what I did. You can get 10 pretty easily just by playing through the game normally (complete certain story missions, have nobody die during a mission, etc.) If you want her on the first playthrough I'd guess you'd have to go a bit out of your way for 1 or 2 (play a bunch of skirmishes to up your kill count and get those medals earlier for example) but it still shouldn't be that hard.
