I'm still wondering why some translate ドランバルト (Doranbaruto) as "Doranbolt" instead of "Doranbalt" (バ is "ba", not "bo"). Or the surname of Gray's Edolas counterpart, ソルージュ (Soruuju), as "Surge" instead of something like "Sorouge".

And I remember when everyone on the Fairy Tail Wiki flipped their lids when Kodansha translated "Tenrou Island" as "Sirius Island". You know, because "Tenrou" (Heavenly Wolf) is the Japanese name for Sirius, the Dog Star.
