In short:
-Araragi discovered he couldn't see himself on a mirror anymore.
-He chose to call Yotsugi for help. To be safe, he sent his sisters at Kanbaru's.
-He found Yotsugi together with Yozuru.
-The two explained Araragi that he had turned vampire (=having his blood sucked by Shinobu) too much times, and now he was close to become a vampire forever. If that time came, they would have had to exorcise him.
-Suddenly, they receive a call from Gaen from which they learnt that his sisters and Kanbaru had been kidnapped.
-the kidnapper was called Teori Tadazuru (Tadazuru = sine. Yozuru = cosine. Yotsugi = cotangent), a stray specialist who had cut his ties with Gaen's network, so he could exorcise Araragi even despite he had been deemed harmless. Tadazuru wanted to have Yotsugi, he left Gaen's group because of that.
-They found out Tadazuru was waiting for them at the usual Kitashirohebi Shrine.
-Araragi and Yotsugi go ahead.
-Araragi sent Yotsugi to check for the hostages while he temporized by talking with Tadazuru (and he couldn't turn into a vampire anymore).
-Tadazuru told Araragi he had noticed a mastermind behind the events, and urged him to find Oshino. Then he let himself be killed by Yotsugi. "I killed him because I am a monster. Don't you ever become like me, devil brother"
-Araragi understood that the purpose of this story was making him estranged with Yotsugi, who had become a murderer in front of his eyes. He had lost Hachikuji, Nadeko, and now Yogtsugi.
-In the end of the book, Araragi learned his sisters had a new doll: Yotsugi. Gaen sent her there so that she and Araragi could still be friend.
This and other details, like the fact that Kagenui doesn't walk on the ground being the result of a curse (but we don't get to know by whom).
