That was my initial thought, but he didn't mention any radiating pain, nor a difference in sensation along the dermatomes. Another possibility is that he has a minor fracture, which might have occurred if he landed on his outstretched hand with a lot of force while getting into the pushup position. Wouldn't be uncommon.
