You probably haven't seen players kiting Kazaak (world bosses) to Stormwind (safe zone), or Varedis to Adal, or bringing debuffs to safe-zone, and all of them came from just one MMO - different mechanic, but those kills players afking in safe-zone. Developers are just human, there can always be something "creative" slip past their attention.
About players killing each others, Akihiko said he wanted to create a world, it seems he succeeded. It's the same in real world.
