As for the rest, I say around 5 to 10 minutes(10 is pushing it for most episodes) are based on manga material. (e.g. Kyoani made a whole concert episode based on roughly one page of 4-koma manga)

Okay, obsessive as I am, I quickly looked through the episodes to see how many are based on the manga and how many are not.
Spoiler for Long and obsessive:
Episode 1 is largely based on chapter 5 from volume 3. KyoAni's most notable additions are Yui playing guitar in the first scene, and the girls dressing up in the animal costumes again.
Episode 2 is based on chapter 4 from volume 3, in which they find Sawako's old guitar and sell it. KyoAni added stuff like going to the hardware store and, more importantly, Ton-chan.
Episode 3 is a KyoAni original, IIRC.
Episode 4 about the Kyoto class trip is obviously taken straight from the manga. KyoAni added some sightseeing stuff and the part where they got lost.
Episode 5 about the Juniors staying home is a KyoAni addition.
Episode 6 about the rainy season is based on volume 3, chapter 8.
Episode 7 is based on vol 3, chapter 2. KyoAni added everything about the fan club tea party, so in this case their expansion of the story was quite considerable.
Episode 8 is a combination of volume 3 chapter 6, and the special chapter with the flashback to the beginnings of Mio and Ritsu's friendship. KyoAni added turtles.
Episode 9, the talent contest episode, is a KyoAni original.
Episode 10, with the Death Devil reunion, is a KyoAni original.
Episode 11, in which they complain about the heat, is based on vol 3 chapter 9. KyoAni added the Ton-chan stuff (naturally, as Ton-chan doesn't exist in the manga)
Episode 12 - about Summer Fest - is based on vol 3, chapter 11.
Episode 13 is based in vol 3, chapter 12.
Episode 14 is based on vol 3, chapter 10.
Episode 15 is a KyoAni original. My least favorite episode, but that's irrelevant.
Episode 16 is based on volume 4, chapter 5. KyoAni added further one-on-one interaction scenes between Azusa and the other girls.
Episode 17 is based on volume 4, chapter 1 (about them trying to come with lyrics, ending up with "U&I"). KyoAni added the not-having-a-clubroom stuff.
Episode 18 is a combination of vol 4 chapter 2 and vol 3 chapter 1.
Episode 19 is based on vol 4 chapter 3. KyoAni added the Occult Club.
Episode 20 is also based on vol 4 chapter 3. KyoAni added a lot with the concert scenes. The scene in the club room after the show was expanded upon as well.
Episode 21 is based on vol 4 chapter 4 (the stuff about their uni choices, and Azusa spacing out, and her worrying that they won't be in the club room). KyoAni adds the stuff about the yearbook photo and Yui's hair.
Episode 22, the valentines day stuff is based on vol 3 chapter 3. KyoAni adds the stuff about entrance exams and getting into the university of their choice - all of which happens in the manga, but differently. This would be KyoAni staying true to the "broad strokes" as I mentioned before.
Episode 23 is a KyoAni original.
Episode 24, about gradutation, obviously happens in both versions but quite differently for the most part. However, the final scene is quite similar (Azusa breaking down and beggin them not to graduate, the four graduates performing a song for her) so I can't really call it a 100% KyoAni original. Although the chapter came out mere days before the episode, I have to assume KyoAni were at least told beforehand what the ending would be.
Episode 25 is largely original, but starts out with scenes based on the opening tankoubon-only chapter of volume 2.
Episode 26 is based on vol 4 chapter 6. KyoAni added all the flashback scenes, as well as the scenes hinting that Jun and Ui will join the club (happened in the manga, but differently).
Episode 27 is, obviously, a KyoAni original.
Final tally:
Episodes mostly based on the manga, with any KyoAni additions mostly being "more of the same" to fill out the episode: 10
Episodes partially based on the manga, with KyoAni changing the focus of/adding significant parts of the plot: 10
Fully original KyoAni episodes: 7
