Indeed, the ending was just perfect. The last suspense moment, where Johan left the bed, left us with more questions: will he begin the massacre again or not? Well, we will never know, after all.
Anyways, Tenma didn't hallucinate about Johan being there. He hallucinated about their conversation; Johan was there, sleeping in the bed.
The only thing I didn't like is that Urasawa didn't reveal Johan's and Nina's true names. It contradicted one of the messages he passed along in the manga, that names are very important things. Oh well. It was more of a curiosity, after all.
By the way, Mangascreener's "extra chapter" was really cool and funny. stephen-sama, I'm really looking forward to "Another Monster"
