That's an interesting question, I believe the results would have been the same considering that it was revealed that Kotonoha actually liked him for quite some time. It didn't take much for Kotonoha to become mentally unstable so I think it was just a matter of time. Maybe if Makoto never confessed to her, still she was in denial for most of the series so it's hard to really say.
