Oh, and, if you don't want to see rape and sexual abuse, I suggest you STAY AWAY from 90% of hentai. That kind of stuff doesn't phase me (nothing really does), but I certainly understand how it could greatly disturb someone.

Well, you must not watch that much anime, because Elfen Lied's violence mostly consists of blood splattering.

The most violent Anime series I have seen till date are:

-- Hellsing Ultimate (That scene where Seras' mother got killed and then rapped right in front of her when she was still a kid.. was crude and traumatizing as hell... seriously I have yet to see another series that would take things this far)

This video gives u a pretty good idea of what I am talking about:

-- Higurashi no koro ni? lol this one was funny too

There was this other one with the pink haired chicks with telepathic powers where people got hacked to pieces, that one was pretty violent too but I can't remember what it was called.

EDIT.. just found the name of that ^ anime a few posts above.... Elfen Lied, yeah that one was pretty shocking at the time

The violent scenes are too few, But it can easily be a contender for most violent anime if madhouse decide to adapt the rest of the manga.

Elfen Lied was quite violent, but since the intention of the manga-ka was to 'question and discuss values relating to the way in which humans divide each other by difference, as well as the belief that atrocities such as those committed by Lucy in the series are strongly influenced by the way in which people are treated by their fellow beings'. I believe it achieved that and did it well, so I found Elfen Lied very watchable.

Gantz had novelty for a while but I grew weary of it. I know that amongst adults it's liked to a notable extent. Also noticed young adult females in Australia seem to like it. Manga outlets say it's one of their highest selling titles.

For me, the two series that felt the most violent and chilling for me were Baccano and Durarara. Graphic violence is one thing, but when it's made out to be cool or funny, then it really causes me to have an adverse reaction. I tried to persist with these series, but they eventually became toxic for me and I couldn't handle it anymore.

I might even had in mind some scenes from the manga when I threw in Shigurui.
I am eagerly awaiting new chapters for quite some time now, it seems that things are coming to an end in a few chapters.

Certain anime from the '80s (especially OVAs) tend to be the most violent and disgusting.
