For anime Yoko Kanno and Susumu Hirasawa wins hands down (for me). Paprika is one of my fave soundtracks! And I don't think I've ever heard anything bad by Yoko Kanno. I don't have any fave video game composers though I should. Nothing comes to mind.

Shouji Meguro because he created so many amazing soundtracks for the Persona series, particularly the Persona 3 and 4 games.

Ritzuko okazaki can be in this list?, for me she it's better composer in anime music.

... I'm surprised no one has mentioned Joe Hisaishi yet

and of course, Nobuo Uematsu is a music god. Plus the composer behind Pandora hearts is up and coming, or so I understand.

Joe Hisaishi (The entire collaboration with Hayao Miyazaki)
Yoko Kanno (Escaflowne, Ghost in the Shell: SAC, Macross Frontier)
Yuki Kajiura (NOIR, Madlax, part of Gundam SEED Destiny, .hack//sign)
Toshihiko Sahashi (Full Metal Panic, Gundam SEED/Destiny, Zipang)
Mark Mancina/Hans Zimmer (BLOOD+)

Games:
Lorne Balfe (Call of Duty: Modern Warfare 2; Assassin's Creed III)
Nobuo Uematsu (Final Fantasy series)
Koji Kondo (The Legend of Zelda series, Super Mario series)

Just a note about Mark Mancina, you should listen to his most recent score for Disney's Planes. The man is back in full strength.

I'm generally a fan of VGM (video game music).

I've been listening to Yoko Shimamura, Yasunori Mitsuda, Nobuo Uematsu (basically anything from FF), Jeremy Soule.
