It all matters where you emphasize it. with the right sounds and right animation, the lack of blood is sufficient.

Imagine Kimmimaro ripping out his spinal with a swooshing sound. It kind of ridicules the original meaning but it get's the job done.

A lack of blood in combination with Kimmimari's static face also loses the illusion that the event is extremely painfull which would add to the gore-factor. Without it, it's not so bad. Just some white spikes sticking trough his body.

Although I doubt it will happen like this, I just thought I'd throw that out for a kick. But like Animizzle said, adding the right sound effects will make it not as gross, but at the same time, f*** Kimi, how are they going to handle Sakon/Ukon's fight [at the end].

EDIT: As far as the bone sound effects, I am a punk when it comes to sound effects, I even flinched on the Majin Buu neck cracking sound effects.

It could actually work. Picture it, they don't reintroduce Rock Lee yet and they make up some excuse as to why Gaara hasn't appeared while the other two [Kankuro and Temari] has appeared. They could say, Gaara is still recovering from the Naruto incident or something. It would leave room for a way to make another filler arc as to reintroduce Rock Lee and Gaara or something. Although I doubt it will happen like this, I just thought I'd throw that out for a kick.
