Vice and Lutecia dying together is a near impossibility. Not because of any 1337ness from Vice, but rather because Seven Arcs values its lolis than its men. If Vice's dying, he'll be doing it alone, I fear

Voltaire and Hakutenou crashing into the barrier is very possible, or better crashing onto Otto, the sub-commander of the Numbers team, killing him and producing almost the effect that the Forwards will suffer from a dead Tia onto the Numbers team, especially his twin Deed imho.
But then again it's equally possible that the giants will only crash the barrier and everyone within will pop out dusty but miraculously unhurt.
Fate and Nanoha achieving limit break. Heck, which shounen show doesn't do that? The only question now is how they'll suffer from the sub-effects.
Hayate goes berserk after learning that any of her knights died is highly possible. SS-class spell of world destroying effects from Rein I's darkest memories coming up if this happens. If she wasn't so badly shafted, I would also like to add, "Let the world despair."
Though imho cold and controlled rage is much more interesting to see, and more terrifying. Imagine Hayate torturing any of Scaglietti's associates unfortunate to fall into her hands (that's what I'll let her do if I'm writing
).
Subaru breaking Ginga... Not that I want to see this, but unless they are really pushing the befriending button a bit too much, methinks Gin-nee will only regain herself with a broken body and a broken sis' heart. This will in turn create a true Terminator Subaru that will eagerly seek and destroy any Number available (strangely, Otto comes to mind again).
Though since Gin-nee is a STKJ, unless the damage is lethal (the chances of this is rather high too, I admit), she would live. Since Seven Arcs likes to pander to fans, Gin-nee's popularity should be enough to protect her from the Grim Reaper.
And as our friends have pointed out previously, MSLN has yet to see causalities in the main and more prominent supporting cast. If the men can survive, surely Gin-nee will too!
