Well for the cheating death thingy, the easiest answer is given by RNR. To be more precise by the resurrection crystal... that's because you still have 10 seconds after your HP-bar hits 0, exactly in those 10 seconds you can use the resurrection crystal as well as attacking Heathcliff. We even have seen that several times in the anime that after the HP-bar hits 0 the ppl still spoke to others, so why shouldn't they be able to attack?

This is also why Kirito and Asuna survive. Ten seconds hadn't passed, and the clear conditions included 'fixing everyone's health at maximum' .. TECHNICALLY, the last boss lived too. He just fried his brain through a different method.

The fact that Kayaba helps Kirito later on in ALO arc by giving Kirito his GM account access and that they actually converse with each other after the fight(the part where Kayaba give him The Seed) should already made it quite clear that he succeed. And DJlame yes he did, thus still frying his brain though.

SAO is like playing Ultima Online with a gun strapped to your head.
ALO and beyond is Netrunner where the Net's source code is open source, freely available and being set up by MMO nerds. And you're probably pretending to be some kind of elf. A lighter tone is more or less NEEDED.
