Okay it's not the same, but I am pretty sure this is because of my English patch, since it uses SEEN from the original LB (or does it? I see untranslated lines in the common route so maybe it's not the original LB SEEN, it's the EX SEEN??) in which you can't possibly play Refrain on a fresh install... But if anyone has the English patch and knows how to do this, could they help me out? Thanks!
It's not properly explained and I myself don't see it as a good excuse for a bad ending. But if I were to take a guess it's basically related to the fact that by finding out about her class and how she is treated there, she believed Riki and the others would start treating her differently so she nips it in the bud before it happens.
Again, not sure as this is the only explanation I could come up with.

While I to don't know exactly how Haruka would brake...I attribute Kanata's "unnecessary" behavior to a simple mistake on her part. Perhaps in her own way she believed it would have the opposite effect of ultimately being helpful to Haruka rather than making things worse. She still human, she still makes mistakes and to me, this would actually make the character even better and more human, adding flaws to her character that honestly...any person could have.

I always thought it was Kanata that did it as she reached a breaking point or perhaps one of the student council people with a grudge...but the Kyousuke theory isn't bad considering it wouldn't be the first and only time he helped out during a route.

We've talked about this before...but my theory is that the Dream World has the ability to copy the real world, even things that are beyond the knowledge of those that created it. Thus Kanata would be a mere copy of her real world self (who was not on the buss) but still retain the knowledge and personality of her real self.
I just read a good portion of her route lately. It gave a lot of background on what happened in their childhood and how things became how they are now.

It cuts to a kendo match between Kanata and Haruka. Haruka fails to hit Kanata and hits the floor. She stands back up breathing heavily. Kanata think to herself, 'It's frustrating isn't it, Haruka? To be called a unneeded child.' Kanata understands that Haruka has been suffering from not ever being able to be like Kanata and to be betrayed by her. Kanata wanted to be at the very least Haruka's ally, but no one here would protect them. It was no longer an act of bad relations anymore. The act became reality.

Spoiler for The conclusion of their childhood:
The deal is sealed, and Kanata meets her two real parents which she greeted with "Hajimemashite (a greeting used to indicate it's their first time meeting, which Kanata finds strange/ironic)". Her parents apologize to Kanata for not being able to help her. Kanata explains that Haruka hates Kanata and her real parents. Her mother asks why Kanata is hated as well.
Kanata did not answer the question, but eventually says "The continued act became reality."
Kanata asks for her parents to live with Haruka, but when her mother asks "Kanata, what about you?"
Kanata "We may never ever meet again. I'm not allowed to tell Haruka. Only until the short period up until the end of summer. A [The fun family game]. That child might not accept it as fun, but her dream was to live with you two. That's why, no matter what happens, please say you're having fun."
Even if Haruka will reject it, Kanata wants her parents to say they're having fun to Haruka.
Kanata "She was never been told that it was fun to be together. That's why, no matter how awkward please say it's fun."
Her mother accepts the request. Kanata finds her own request odd and thinks that they cannot talk naturally. Wondering where the thing that was stolen has gone to.
Her mother holds Kanata's hands, and Kanata notices how her mother's hands could not be depended on. Kanata felt strange as to how no tears had fallen from her eyes, because she has been a doll for a long long time.
Kanata asks that her parents to say that they are happy to with Haruka to Haruka, no matter how awkward and unpleasant the situation, since the two sisters did not understand true happiness. She tells them that Haruka will stay with them. Her mother questions what about Kanata? Kanata says that she can't, and that she will only be able to see Haruka in the short time they have at school. Kanata believes that even though Haruka will be bullied by her, it's better than living over there. She believes that Haruka may be able to make friends, as well as obtain something that Kanata herself cannot grasp.
Coincidentally reread Kanata's route recently, just adding a portion about the hair ornamentsthey were talking about:
(note: the discussion with their parents was only with their Father. Their mother was bed ridden at that time).

Kanata and Haruka were put in competition all the time and their relationship sure went awkward, to the point Kanata had to use some tricks to let Haruka have something she earned.
For instance, Kanata got a set of marbles, which cought the fancy of Haruka, but the latter doesn't want to accept any, even though Kanata offered her half of it.
Not knowing what to do, Kanata used the complete opposite approach and pretend to lend Haruka only 1 marble, but she ought to give it back when Kanata sees it fit.
Over time, the competitions done by the Saigusa became quite taxing. Meanwhile, Kanata receives a weird package with a "Saigusa Kanata" on it. She only opens it much latter to discover 4 hair ornaments. She was puzzled at first but realized with the date that it was intended for both her and Haruka as a birthday present, probably from their mother.
Of course, Haruka declines the offer since she believes that Kanata earned it fair and square.
Kanata went ahead and tells her that she won't bully her or anything, and Haruka is relieved to learn that her sister didn't forget their promise and that she didn't truly hate her, that it was all of a charade until they finally can meet their parents.
Kanata then learns that Haruka was holding off all this time, so Kanata can move forward, as times to times Haruka managed to win, but never took the opportunity to be nasty. She took herself the role of the "bad girl", as she believes in Kanata that she can achieve the "good girl" part. This is also part of their promise, so she decided to hang in there.
In the end, Haruka accepts the hair ornaments, and Kanata agrees to wear her own 2 hair ornaments. And soon enough, Kanata had to revert back to the "bully sister" mode as some relative was coming.
As lildevilinhell explained though, the hair ornament became their bound but also the reason why they were drifting apart. More accurately, at that time, Kanata was wearing only 1 pair of hair ornament, while Haruka was wearing both she had. So the adults told Haruka that it meant that Kanata was destined to be the first, while Haruka will remain second forever.
That said, I must have missed a detail but...

I'm pretty sure it was never mentioned unless I missed a specific line, but Kanata herself she confirmed she never threw it away.
Suffice to say, reading it again, I think this route is definitely a mandatory read in order to flesh out both characters and give proper answers to everything that had occured in the past. The only problem of that route is how longwinded it can be (not as "beating around the bush" as Haruka's however), and the conclusion is a bit "too easy" but fits quite well the series, whereas Haruka's route was a tad... too disconnected from the LB in general.

The trouble comes in having to replay the whole game again just to play Refrain... my saves from when I completed it are on a computer I don't have anymore.
It is not pressing enough for me to do that, I was just wondering if there is a way to play Refrain if you answer "Yes" to the beginning question. And anyway, shouldn't Key have put in some mechanism to play Refrain again if you answer "Yes"? Because otherwise you would have to play through all seven girls' routes again, but I doubt that is possible because afaik Rin 1 is forever locked when you complete it and would the game still keep count of whose routes you finished after you complete the whole original game?? Seems like a waste when Rin 2, the rest of the girls, and the new heroines you can get right off the bat by answering "Yes".

Oh, so I remembered the "pretends became reals" from Kanata's route? Yeah...come to think of it I did finish her route first in playing EX before stopping.
Spoiler for Summary of a portion of what happens in Kanata's route:
In her side of the story, Haruka managed to clear her hatred of Kanata by herself without Riki's help from the very beginning **(This is actually false... Will be explained later). Because Haruka remembered the promise
that she made with Kanata from when they are little and what Kanata's true intentions were.
Kanata is slightly more softer in her route and tsundere-ish with a mix of Kuudere.There are some pretty funny scenes where Kanata gets startled by Haruka calling her Onee-chan or Kanata. We see Kanata teaching Haruka how to cook and shopping with Haruka and Kud. The reason Haruka is trying to hard to talk to Kanata was because Haruka was to be transferred to another school.
Riki has been helping out with A-chan's work. I think it was something about to let Little Busters continue club activity. Riki eventually somewhat gets along with Kanata.
Then, there was an incident where in the courtyard, there were two girls. One of the girls was forcibly trying to get the other girl to let her borrow an alchohol lamp that is used for chemistry experiments. While they were struggling with the lamp, it fell and lit a fire. Kanata walks in about to ask what's going on only to be startled by the fire. She quickly takes off her coat and proceeds to try to put out the fire. She sees Riki and tells him to quickly get water to put it out. After the fire is out, she interrogates one of the girls.
Her voice turns dark, and questions the girl if she was doing this bully someone else. The girl tries to defend herself, but Kanata snaps, not allowing her to even talk back. Kanata threateningly tells the girl that she should experience it first hand herself. Riki interjects and tries to explain the situation since he saw a little of what happened before hand. But after Kanata calms down, Riki notices that Kanata's clothes were soaked and atrocious dark red marks could be seen on her arms. People watching the scene begin to whisper and gossip about the marks, and Kanata begins to stiffen and realize the situation. Kanata quickly turns pale, panics, hides her face, and makes a run for it. Riki picks up Kanata's jacket and chases after Kanata leaving A-chan to deal with the rest of issues.
Once he found Kanata in the backyard, he sees Kanata on the floor crouching, holding her throat, coughing, and vomiting. Riki hurries over but he ends up staring at her arms that look as if they were covered in burns. Riki tries to rub her back. Kanata screamed while coughing, "Don't touch me! Don't touch me... Please don't touch me. Don't come any closer. Don't get close to me.".Riki realizes that Kanata did not even recognize him at all. Her eyes were cursing someone else and glared with hatred. He backs off and tries to reassure her that he won't come near her or touch her. Kanata continues on vomiting, and Riki realizes Kanata's arms are trembling in panicking fear of something scary. He keeps calling her name until Kanata becomes emotionally stable. Once Kanata was finally able to recognize him again. Riki places his own coat onto Kanata. Kurugaya eventually came over and saw vaguely comprehended what was going on. Kurugaya tells Riki to let her take over from here.
Kanata asked if it was the first time Kuruguya saw these marks before. Kurugaya said it's not her first time seeing it, stating that it is common anywhere you go. Kurugaya states what is normal, what is not normal, is something that happens often. Kurugaya apologizes to Kanata, while Kanata asks why she's apologizing.Kurugaya asks if Kanata remembers, but Kanata says that Kurugaya is not involved with it. Kanata says "Even if this is not normal, to me this is normal. This is nothing you have to concern yourself about, because this was how I was born to be. That's all there is to it. Because I realize that, that's all there is." Kurugaya replies "How conscientious, you didn't have to understand it to that extent. I won't ask any further. What you really need to do now is return to your room, take a shower, and sleep. I'll escort you there." Kanata realizes that Naoe's jacket is covering and she feels embarrassed and in pain.
Later Riki learns from Kud that Kud already knows about Kanata's marks. Kud states that normally Kanata is fine, but when she is not prepared to see it, she panics (this part I wasn't sure if Kud meant Kanata is not okay with other people seeing it or she has trouble looking at it herself.) There was one time Kud tried to push Kanata to let her take a bath with her, her marks were seen, and Kanata went into a panic similar to what happened in the courtyard. Kud wants to think that those marks were caused by an accident, but gradually her words show doubt that those scars wounds were an accident. She begins to think that it was from someone else but stops partway her sentence.Kud bows and asks Riki to save Kanata, because Kanata has been having painful nightmares and is most likely still suffering even now. Kud feels she isn't of much use, since Kanata won't talk to her about it. Kanata told Kud that "Saying that the wound is not frightening is more than enough". Kud feels helpless at how she can't do anything to help solve her precious friend's worries. Riki states that he will help Kanata and says that Kud has to help as well. Kud objects, but Riki reassures that Kud can help Kanata.
The more the time comes closer, the more Kanata becomes more closed off and tries to push everyone away. She becomes similar to the persona that you see her in Haruka's route. She doesn't rest at all. Eventually she begins to overwork herself to the extent she even came late to class. Her stress keeps building up until she finally collapses in the rain in front of the girl's dormitory while practicing kendo (Kanata is the ex sub-captain of Kendo club, although she hates kendo), where Kud finds her. Kud hurries to find someone (eventually going to Riki's room) to help Kanata. They brought Kanata to the nurse's office.

I called her name. There was no answer.
Riki “Futaki-san?”
There was no sign of movement from the other side of the curtain.
Kanata “...What?”
It was a hoarse voice. I opened the curtain.
Riki “Futaki-san... Are you alright?”
Futaki-san placed her hand up to her forehead. She gazed at me absentmindedly.
Futaki “.......”
Riki “Do you remember? You collapsed in front of the dorm.”
Futaki-san didn’t respond.
Riki “I heard that lately you haven’t been sleeping. You must be tired, that’s why...”

Up until earlier, Haruka-san and Kud were watching worriedly.
Riki “There doesn’t have to be a reason to worry about someone. That’s why I’ll worry about you.”
Futaki “You may be right... however leave me be.”
Riki “Why do you have to say this? It seems to me Futaki-san is simply being stubborn.”
Futaki “Stubborn? I guess I am stubborn.... When I was elementary school.”
Riki “Eh...?”
Without inquiry, emotionlessly muttered suddenly.
Futaki “I had a classmate. A child that left little impression. She always stayed in corner of the classroom. Couldn’t do any exercise or studying. She doesn’t stand out. There’s always one of those people right? During the class rearrangement in April, always casting their eyes down. That girl was that type of kid. That’s why she didn’t have any friends, that type of classmate.”
Riki “Who’s this story about?”
Futaki “It’s definitely not about me. But I’m involved in the story... I was the only one who talked to her in class. Maybe it was because we both had an excuse to wear a long-sleeved jersey. She and I always wore long-sleeved jerseys.”
Futaki-san’s lips raised on only one end.

Futaki "I've always done everything by myself. I'm not like you who was always protected. I didn't have any allies. A hero of justice never appeared! Not even a person who would hug me, not even a person to comfort me. Even though my beloved little sister hates me!"
Futaki shouts as she clutches the sheets.
Futaki "For me.... what else is there to even to wish for. It's not even a great wish. I just want for things to continue as it is now. Despite that, you say you're worried? What is there to even worry about?! You should worry about this everyday life that may soon end! What happens to me doesn't matter!"
Futaki-san stared at me.
Riki "It matters. I may be a single straw. Maybe just one unreliable straw. But I, to me, it seems like you're in danger of being swept away."
Futaki "....!"
Riki "As much as one straw is you can still reach out with your body. That's why I don't think it's trivial. Haruka-san"
Futaki "Eh..."
Riki "Haruka-san asked me. Don't you understand?"

Spoiler for Full Kanata background story leading to up to what is in the present:
When they were younger, whenever Kanata won, we see that she gets gifts for her achievements. On the other hand, Haruka was abused and often came to see her sister. This time Kanata, received a large amount of marbles. Haruka walks into the room despondently. Kanata tries to talk to Haruka. She finds out that Haruka didn't receive any gifts. Kanata tries to split the gifts she received to Haruka, who swatted her hand and refuses to accept them. Kanata asks if it's because Haruka cannot have them because it belongs to the "winner". When this occurs, Kanata resorts to the usual "Haruka is stubborn huh; well then, I'll stop giving you it."
Kanata questions in her mind 'Why are you making such a face? If you want it, I'll give it to you. I'm fine with giving it all to you. If it's Haruka, I'll give you everything.You look as if you're about to cry. Anything else is unnecessary.As long as I have Haruka.'
Kanata continues on her usual, stating that she'll give Haruka one and that she'll take it away if she ever wants it back. This was the only way to make Haruka accept the gift.
After some time, Kanata is starting to notice Haruka is backing away from Kanata when she comes close. A while ago, Kanata received a package sent to "Saigusa Kanata" which had 4 ornaments (the one's that Kanata and Haruka wears). Kanata wants to believe that they're from their real mother. She tries to approach Haruka and give her half of them. Claiming that they're from their mother.
Haruka and Kanata made a promise.
The promise that Haruka and Kanata made to each other was that after they manage to escape that household, they would hand hands together and go see their parents. Until that day comes, they would have to continue to pretend to have a "bad relationship as sisters". Haruka encouraged Kanata to continue rising up as an heir while Haruka herself would continue to be the good for nothing so that Kanata can change the system within the family.
As a result the play of the two sisters with bad relations continued, as Haruka suffers emotional and physical abuse, Kanata suffers from her heart being worn out from unable to help her sister. If Kanata shows any sympathy for Haruka, Haruka will get beaten as a backlash. So Kanata keeps trying to hold onto the lie as a way to protect Haruka. They were both clinging to a sliver of hope to their dream.
Around a cut scene into this flashback when Kanata collapses, we find out Kanata hates the rain, because she has seen the pool of blood on the asphalt many times.
On the day the heir was decided and Kanata was notified by phonecall, it was raining on that day as well. On the phone, Kanata asks what will happen to Haruka, stating it's not out of sympathy but curiosity. Haruka will be entrusted to the Saigusas. Kanata in the midst of thought of what would be the best thing to do. She ended up in front of the Saigusa's house. She hesitated and lacked the courage, but she saw a shadow that she hadn't seen for a long time. As she was about to call out Haruka's name, but it was broken off. Haruka looked at Kanata emotionless as if she were dead.
The dialogue from adults, "Do you why there was two hair ornaments? You have no choice but to be second. This is Kanata's will. It been this way since a long time ago. Don't you know it's decided? It seems Kanata already threw away one of them."
Basically, the adults are mocking Haruka's position and status and telling her Kanata's also behind this. They also tell her that Kanata threw away one of the four ornaments that Kanata gave to Haruka on that day they made the promise. In reality, it was the household that forced Kanata to get rid of one of them, since the household wants Haruka to stop believing in Kanata. From the situation, it seems the adults threw one of ornaments that Kanata had to give up in front of Haruka as proof of Kanata's betrayal. This is why Haruka is seen wearing 2 ornaments normally, while Kanata only wears one presently.
Kanata tries to say "That not-". Kanata thinks, 'I never thought that. I didn't throw it away. I wouldn't throw it away!
Adults continued to mock Haruka "She (Haruka) clung to it so sincerely, and she didn't even realize she was tricked."
Kanata's mind continues to cry out it's not true, however... Not a word comes out of Kanata's mouth despite her desperate pleas from her heart for Haruka to not believe them. She tries to ask herself what she should do desperately. Haruka looked at Kanata with the same eyes that she gives the adults, mixed with scorn and hatred. Kanata from the bottom of her heart was crying out that it's not true and hopes for Haruka to remember the promise.
It cuts to a kendo match between Kanata and Haruka. Haruka fails to hit Kanata and hits the floor. She stands back up breathing heavily. Kanata think to herself, 'It's frustrating isn't it, Haruka? To be called a unneeded child.' Kanata understands that Haruka has been suffering from not ever being able to be like Kanata and to be betrayed by her. Kanata wanted to be at the very least Haruka's ally, but no one here would protect them. It was no longer an act of bad relations anymore. The act became reality.

The wall is not even enough to protect from the wind and rain. It was not a place that person can live in. There were footsteps following Kanata.
Kanata snaps at them and demanded for an explanation without turning to look at them. She tries to approach Haruka, which shrinks away from Kanata.
Kanata hears a sorrowful scream from Haruka saying, "No...No... I... won't fail anymore... next time, next time... I'll do my best. I'll try hard... harder.... harder!!!"
Kanata stands up with fist trembling screamed "Why... My... What did to my Haruka!? You all promised me, that if I worked hard for all of you, that you wouldn't do anything to Haruka! You told me that! What is this? What is the meaning of this!? Answer me! Answer me! Why are you silent!? ANSWER ME!"
No one said anything, none of the people of Saegusa househeld said anything.
Kanata's uncles made an excuse, saying that they couldn't help it, since they needed something to vent their emotions on. The household members implied that the condition of her sister was actually Kanata's fault. The need to vent was caused by Kanata trying to get rid of the traditions that they have long clung to. Kanata tries to appeal to the townspeople and the members of the household that there must be some of them that must realize these traditions are strange. Kanata wanted to change the customs, like how Haruka told her, even though Haruka sees her as an enemy now. Kanata didn't want to forget the promise.
In desperate long pleading, not a single person said a word. At that time Kanata realizes that no one was going to side with her and Haruka, and that no one was going to help them. They told Kanata that she was not a representative, not until a suitable marriage is done. Kanata questions since she thought this was supposed to be after she finished university. They continue to say there's no reason to change the customs, asking isn't Kanata already happy with everything she has received from them. Kanata did not feel happy. She was under surveillance, forced to do what she didn't want to do, not even allowed to rest. There was no happiness, it was painful. But that wasn't what was painful. Her sister has to suffer. Why is this happiness, she thought.
They questioned Kanata's motive and asked whether it was for Haruka sake and to save Haruka. Kanata knows if she says that, Haruka will be removed. Kanata didn't want her sister to be out of hands reach. She lies again and says that breaking these traditions will help build their reputation back and have the people accept the household once again.Kanata begins to think of whether to ask for help or run away. But comes to the conclusion that running away is only something that's easy to say. As if running away would save them, she thought. If it were that easy, then why is it on television and newspapers there are dozens of children being swept away.
Eventually the discussion was lead into ultimatum. Kanata is eventually cornered to admit that she wants to save Haruka. They told her that if she wants to save Haruka, Kanata must stop her attempts to remove the customs and give up her own future. Kanata questions them that after breaking their promise, how can she believe what they say?
Kanata asks what will happen if she refuses.
They told her, "As it is now, your sister.... Who will be first? Your expectations being realized or that child breaking."

Kanata screams at them, "...You. cold-blooded beast!!! You who despised us for being good-for nothing, those words suit you best!"
Household "Are those your true feelings? Fufu, are you that man's daughter? However, it seems your younger sister is obedient no matter what's done to her. I wonder if the resemblance is in how your mother and father couldn't do anything but run away?"
Kanata "Don't insult my parents."
Household "Insult? It was justified assessment. Whether you parents were the worst, the truth is that they abandoned you and ran away. By no means am I blaming them. Everyone prioritizes themselves first."
Overwhelmed in disgust, I felt nausea.
Household "Even Kanata... it's the same for you, isn't it? When I said, you can take her place, you didn't nod did you?"
Kanata "That was.... that was."
I looked down. That was my weakness. That's why, I thought I would change it. That I did not nod. That was my mistake. I wanted to apologize. But, that's no longer possible. If I can't do that, I want to save that child. Even with a detour, I'll make small changes and someday. With that child... My ideas went in circles.There were no exits.
Household "What is Kanata's true desire....?"
A gentle voice. That voice slips into the innermost depth's of my heart. I looked around my surroundings. No one looked away, I couldn't respond. Once I've lost that child's smile, I am already alone. There was no help. There was no salvation. Because I'm not an adult. Or maybe because of my birth. Many people were hurt. I was only one of them. Even if I struggle by myself, it is only a powerless child. There is no way to stand up against it.
Household "You want to save you sister right?"
That's right, my desire is... What I wished for a long long time ago.
Kanata "...If I agree..."
Looking at the floor, I saw the ground. I could hear the sound of raindrops strongly.
Kanata "If I, abandon, my will.... Will you really save that child? No longer, toward that child, absolutely.... won't do anything...?"
Household "Yes"
Kanata "Will you let her go to school?"
Household "Aaa, that's fine."
Kanata "Will you let her live with mother and father?"

Looking up, a cold remark fell on me.
Household "That is not allowed. Don't do that."

I tightened my lips. I'm not even allowed to hold hands with her. That's what it means to throw away my future. Yes, I understand. But, Haruka will be saved. In that case, isn't that fine? If I become the sacrifice, Haruka will no longer have to do her best. There is time. Certainly. But if Haruka completely breaks, there is no point in her trying her best.
Kanata "If it's now... I can save Haruka."
It felt like a hope. If I presevere, Haruka will be saved. Then, I have to preserve the status quo. This is what they're saying. That is a very, very good method, I thought. I was tired. That child's smile was unreasonably stolen. No... I was the one who stole it. Where did that stolen smile go? If it's out of reach, I've lost my reason for being here.
Kanata "...Haruka."
My important Haruka. My precious Haruka. My little sister.
Kanata "... I understand."
I said. I should have done this from the beginning. It would have been best for me to be doll. I couldn't be a [good older sister]. We were only two sisters. Ever since we were little, we've only had nothing but cruel memories, but we still had fun. For the sake of that child's smile, I have come a long way.
Kanata "I understand. I'll... be at your mercy."

The deal is sealed, and Kanata meets her two real parents which she greeted with "Hajimemashite (a greeting used to indicate it's their first time meeting, which Kanata finds strange/ironic)". Her parents apologize to Kanata for not being able to help her. Kanata explains that Haruka hates Kanata and her real parents. Her mother asks why Kanata is hated as well.
Kanata did not answer the question, but eventually says "The continued act became reality."
Kanata asks for her parents to live with Haruka, but when her mother asks "Kanata, what about you?"
Kanata "We may never ever meet again. I'm not allowed to tell Haruka. Only until the short period up until the end of summer. [The fun family game]. That child might not accept it as fun, but her dream was to live with you two. That's why, no matter what happens, please say you're having fun."
Even if Haruka will reject it, Kanata wants her parents to say they're having fun to Haruka.
Kanata "She was never been told that it was fun to be together. That's why, no matter how awkward please say it's fun."
Her mother accepts the request. Kanata finds her own request odd and thinks that they cannot talk naturally. Wondering where the thing that was stolen has gone to.
Her mother holds Kanata's hands, and Kanata notices how her mother's hands could not be depended on. Kanata felt strange as to how no tears had fallen from her eyes, because she has been a doll for a long long time.
Kanata makes this request, since the two sisters did not understand true happiness. She tells them that Haruka will stay with them. Her mother questions what about Kanata? Kanata believes that even though Haruka will be bullied by her, it's better than living over there. She believes that Haruka may be able to make friends, as well as obtain something that Kanata herself cannot grasp.

Also no, Haruka didn't know about Kanata getting married (their cousin questioned Kanata about that, and Kanata states Haruka doesn't know and that's for the best), although Haruka had a hunch, considering it was the Saigusa and Futaki at work (she probably knew the fate of the heir, but obviously not the fact it would occur that early.

I'm pretty sure that this scene was showing both Kanata and Haruka "souls" before getting send to another loop of the dream world, which implies that no matter what happens, they will be reunited as they are supposed to be.
That "rescue" mission was... odd though: I mean I assumed they managed they could stay together after fending off the Saigusa goons back in front of the school entrance, but then we got that random wedding scene. At least like I said, it reinforce the "LB gang" vibes, although it was a tad random.

Yeah... I thought that the ending was strange as well. I thought that after that fight in the school entrance, they were going to make Kanata regret her decision, instead of still keeping in the family to marry to another relative. I couldn't imagine Kanata obediently go back to the family after she asked Riki and Haruka to run away with her. So, it felt like an alternative ending instead of a continuation.
The only conclusion I can make with this scene is that this is the aftermath of yet another loop for Kanata, representing what Haruka meant that they will be together over and over again.
And considering their memories from previous routes are kept intact to a certain extent, there isn't really any more grief between the twins, although the Saigusa is still a pain, so the LB'd wait for the wedding ceremony to barge in and save the day.

Refrain didn't state clearly what happened to every girls in reality: whereas heroines like Yuiko don't need really any "real life" conclusion (considering their problems had happened and they went ahead with that), other heroines like the twins, Aya and Kudo might need more clarification to what happened to them/stuff related to them... well save probably Aya, considering the "obvious" consequence...
I haven't read the other EX routes, but it felt like the main point of the epilogue was that all the heroines (in original) managed to properly move on. We're probably just expected to think that they were to properly get past the obstacles in real life. It's a convenient method, although it would be nice if they went more in detail.
That being said, I can't help but sympathize whenever Kurugaya makes a complicated expression in the epilogue, since it seemed she retained her memories, while everyone else is not nearly as troubled by them.

Komari, Kud and Mio's trouble was hugely a mental problem, so they could simply came off stronger than before. Kud's parents remain dead, but she doesn't feel as stressed as before.
What's hard to do is Haruka's. She may have remembered the old promise after refrain, but she still have to work it out with Kanata irl because she shouldn't be in the bus back then. But, considering that the whole dream world could only exist in minutes in real time, a few hours at most, it could be that Kyousuke forcibly took Kanata's consciousness for that small period into the dream world, so when all that ordeal has passed, Kanata remembers it, even if subconsciously.
Damn, suddenly I can picture that the first original bus trip was filled with gloom and dark air due to everyone's personal problem, actually I'm surprised they could go in the first place. oh wait, the first bus trip was school planned wasn't it?

From what I know, two routes out of three have been translated. The third one is pretty long, though, so it might be a little longer before the full patch is released. I'd recommend just playing the original, and moving on to EX when it's out. You can effectively skip to the new material then.
