Hmm, maybe it's to avoid confusion? Since he thinks mikoto's mom is her sister and if he calls her misaka when her mother also present, it will make a confusion on who he calls(whether her "sister" or mikoto herself).

Touma only calls Mikoto by her family name because it's a formality, just like how he calls Fukiyose Seiri "Fukiyose-san." It's also because it's so formal that it shows how Touma is so clueless toward girls.

I wouldn't say its because he is clueless, just that he doesn't know them well enough to be using their first names. If my understanding is right, its kinda a big deal in Japan.

Plus, if you take into account his memory loss, he has known most of them what? two months? Less in most cases. Who knows, he may now be calling someone he used to call be their first name by their last name because he doesn't know that he didn't before.

Technically, Index and Mikoto should be more formal towards Touma than he towards them. How often have we heard Index or Mikoto call Touma "Kamijou-san"? What I'm saying is that if the girls can be so casual towards him, then Touma can as well.

.....I rarely hear any of the girls call him kamijou or rather the only one I know who calls him that is Styl and he calls touma by the whole name "kamijou Touma".

As for the girls its always touma.
