Nanoha and Fate didn't really do anything about that bit, StrikerS confirms that Rein Zwei wasn't born untill several years later, so up untill recently the 'when' part of the 'when and how' was unknown.

Its an extra, Nanoha's "full transformation". Fate's is on 8. Subaru's is on 4, Tiana 5, Caro 6, Elio 7 (those last 2 might be switched I can't remember).
