I got the impression that the Covert Ops Head and 2nd Captain positions were separate until Yoruichi ran away with Urahara, when Soi Fon took both, but it makes sense that Yoruichi would be a Captain, but no-one refers to her as one... Or do they?

Yeah, Mayuri and Aizen call him the previous 12th Captain and Ikkaku has a flashback of him wearing the 12th Captain's coat. I thought Urahara = ex-12th Captain was a given?

Soifong is both cover ops leader and 2nd division.yourichi was only the covert ops leader, not a captain at all. After yourichi left, soifong took over her role as covert ops leader and she also got into a position of being a captain of gotei 13. Gotei 13 and Covert ops are seperate. From what he was saying, the crest was being kept in a vault in Soul Society and Ukitake had to unseal it.

Concerning the swastika, while all of that is true, it is irrelevant to answering the question. The swastika has counter-clockwise arms. The symbol with clockwise arms is the manji, or ban. They each have different meanings. In this thread, please don't post unless you are asking a question, or answering one. I'll leave your post alone in case someone is interested in the (only vaguely relevant) history of the swastika.

The top symbol are simply the numbers from 1 to 13. These symbols for the numbers have been largely replaced by the western style numbers today, but before the current era those symbols were used.
