She didn't give him false information, she simply withheld the information that would have ended in Tatsuya not being interested in interfering. She knew about the fail-safe that prevented the dolls from harming the students. What she didn't realize (and the Kudou family by extension) was they were playing with fire to begin with because the parasites could get out and then possess the students. The reason Yakumo withheld that information is because he wanted to determine if the Kudou and the 101 division understood that risk and were still willfully withholding that information from Tatsuya. So in reality it went from them using Tatsuya to test their new toys to Tatsuya actually bailing them out.

Tatsuya's blunt attitude really makes me laugh sometimes, and the recent update with his chat with Masaki really puts into perspective the insinuations from Maya and Kazuma in earlier chapters about how uncontrollably unstable Tats can be.

When did Tatsuya use it exactly? I don't remember it.
I am guessing it was the attack tatsuya was using against Masaki. The one that was so weak that Masaki just ignored it. Tatsuya just used it to coverup his use of flashcast. Fumiya is doing the same to cover his mental pain magic but Fumiya is actually able to make it strong enough to do some damage on it's own.
If I remember correctly Tats used Phantom blow in one of the preliminary matches in Monolith's code; it is mentioned (listed off) during one of the discussions between Fujibayashi and the Doctor of the 101 battallion observing the matches, when he's 'protesting' that Tatsuya's is cutting too many corners by not using even Elemental sight or Flash casting...
He toyed with the idea of letting the defender pass, but he ultimately chose to release his grip, twisting in the air to prepare for his landing and pulling out the CAD from his right at the same time.
