Well, I don't remember everything in all the episodes, but I think first we thought that he died in the satellite/leukocyte attack episode, then we thought that he died in the plane (during the virus "activation" before Inori started singing to reverse it), then in the latest episodes he received a wound meant for Shu (and we suppose that now he died). But also as a kid during lost christmas incident the explosion wasn't enough to kill him.

There's still the episode when they blew up an airfield with Gay standing smack in the middle of it and then episode 11 where he was almost slashed in half. So make that 3 times if you count episode 12.

Yes, I really do wonder what caused Ayase's disability. It's true that Hare's void has not been used on a person yet, so maybe it might not affect organics.
I do feel sorry for Ayase if she does not regain her ability to walk soon. She is currently 17 years old, and when she turns 18, Shu will no longer be able to pull her void from her. Hence, she won't be able to move like she did in the past episode.
