But in any event, as long as the option chosen gives Mio-dono the choice as to whether or not her potential suitor would actually interest her or not, I'd be fine with that.

She probably filled out the top part on the 17th, presumably after Nodoka conversed with her about not joining clubs might lead to becoming a NEET. (~3:55) Then when she decided on the light music club, she filled out the rest of the form and changed the date instead of using a new form (either that or the forms weren't easily available anymore (it's been three weeks and everyone else has decided on their clubs)).

Or she put 17 to indicate that she decided a week ago and then thought that she'd better not and be more honest with the date, or the teacher had her change the date to reflect the actual day when she applied (proper procedures and all).

Eh, it's hard to tell with the cross-out/scribble in the way combined with the resolution, but that might have been "19" before she crossed it out.

Dunno, that usually happens to me with months or years... but who knows. Sometimes I won't have any paper I need to write the date on for many days in a row and when it comes time to write the date again I could well be two or three days off.

Of course this is starting to head into overthinking a joke territory, so...

I don't think that's what happened. I think Willen is probably right: she may have dated it on the 17th, but then didn't decide on a club until the 23rd. Of course, that's cute, too, lol. But at least there's a reason.
