Most disturbing anime scenes? (SPOILERS!)
Let's try to tone down on graphic descriptions, and remember to use spoiler tags.
My first choice would have to be the final episode of Berserk -- completely came out of nowhere. And then, of course, the infamous Spoiler:

Anything from Bokusatsu Tenshi Dokuro-chan and the final episode of School Days. Saya no Uta would probably be on my list if it were adapted into anime, but I personally wouldn't watch it. Just having a glimpse at the CGs and summaries of the VN convinced me it wouldn't be my kind of show. No offense at all to any fans of Saya no Uta though.

Gore is not all that disturbing to me. I'd be interested in scenes that make you feel uneasy, like you want to stop watching. I can't think of many animated examples, though.
