Given the fact that Mei knew Misaki was going to die soon, I'm not surprised that the actual death hit her like a sledgehammer. The episode showed us that they had very good chemistry (of course, they are sisters, so that's to be expected) and that she had a positive effect on Mei, who resembled little of the socially awkward, silent and withdrawn self we got to know in the original anime.
I assume Misaki developed acute leukemia, given that the symptoms and her death followed each other so quickly.

Mei before it all started.
Those were getting along really well. Just like any other normal girls, until it went wrong. The ferris wheel part was kinda scary & then it got just silly. There were no horror parts. I felt disappointed.
