That would be because the size is too large for AnimeSuki. It's 64kb and the limit it 50kb. I hope you don't mind, but I resized it for you so this should work here. ^^

Anyone know where I can locate this avatar? Or alternatively, could someone make an avatar as descripted?

Sorry I had to change some of your specifications. Blue couldn't be seen, and white was more... Contrasting.
